Bell-end is a very common (and quite rude) British insult for someone who is being an idiot, acting stupidly, or being generally annoying. While it’s technically anatomical slang, it’s used widely as a generic insult for someone you find particularly irritating. It’s definitely informal and should be used with caution—usually only among friends!

Is it positive or negative?

Negative. It is an insult, although its "severity" depends on the tone and who you are talking to.

When do you use it?
